Heart Failure Risk

Onglyza is a dipeptidyl peptidase 4 (DPP-4) inhibitor approved by the U.S. Food and Drug Administration (FDA) in 2009. The drug was seen as a breakthrough in the treatment of type 2 diabetes because it effectively controls blood sugar without encouraging weight gain.

However, a 2013 study from the New England Journal of Medicine found that DPP-4 inhibitors increased the risk of heart failure by more than 25 percent.

For a two to three-year period, researchers observed 16,492 patients who were randomly given Onglyza or a placebo. They found that 2.8 percent of subjects who took a placebo were hospitalized for heart failure compared to 3.5 percent of patients who took Onglyza.

An FDA advisory panel analyzed the results of this study in 2015 and concluded that concerns about heart failure from Onglyza were valid. The panel recommended that Onglyza manufacturers Bristol-Myers Squibb and AstraZeneca add a warning to drug packaging about the increased risk of heart failure.

The FDA updated the warning in April 2016, announcing the change in a drug safety communication. The warning notes that DDP-4 medications like Onglyza increase the risk of heart failure, especially for patients with heart or kidney disease.

The warning advises patients to contact their health care professional immediately if they experience symptoms of heart failure, including:

  • Shortness of breath during routine activities
  • Struggling to breathe when lying down
  • Fatigue or weakness
  • Weight gain accompanied by swelling in ankles, legs, feet or stomach

In late 2011, the FDA added a warning about pancreatitis to Onglyza’s warnings and precautions. The FDA said that it had received reports of Onglyza patients experiencing acute pancreatitis.

The FDA advised doctors to carefully observe patients for symptoms of pancreatitis and discontinue the medication if any symptoms appeared.

Two years later, a study from the journal Diabetes revealed that DPP-4 inhibitors may be linked to pre-cancerous cells in the pancreas.

Researchers autopsied the pancreases of people taking incretin drugs, including DPP-4 drugs like Onglyza. They found that pancreases from people taking DPP-4 drugs had benign tumors that could become malignant, while those from people on other types of drugs did not have these tumors.
